The symptoms and signs of rape/sexual assault vary with the degree of violence incurred by the victim. Many come to an emergency care facility, physician's office, clinic, or a law enforcement agency stating, "I've been raped." Some report having pain in the pelvic and perineal regions or being choked or restrained in some manner. Others relate the threat of death. Bruising may be noted in any area of the body, not just the pelvic area. Signs of rape include torn clothing, dirt and debris ground into clothing or hair, disheveled appearance, withdrawal, anxiousness, avoidance of eye contact, and bruising, tears, or lacerations around or on the genitals, rectum, or mouth. Pelvic and rectal examination often reveals tenderness in those areas and evidence of trauma. If the victim has not bathed or douched, evidence of semen may be present on the inner aspects of the thighs, genital areas, mouth, nares, or pubic region. Any part of the body, including the breasts, may be traumatized. If the victim is male, bruising or lacerations in the anal region are indicative of sodomy. Bruises and lacerations may also be discovered in the anal region of the female when she has been sodomized.
Diagnosis is made by evaluating the history, clinical findings, and physical examination, including pelvic examination and sex crime evidence gathering. Samples of debris, clothing, saliva, hair, pubic hair, fingernails, and scrapings from under the nails are collected. A urinalysis is obtained, along with blood tests for sexually transmitted diseases, AIDS, and possible pregnancy. Photographs may be taken for documentation of injury. It is recommended that this type of complete examination take place in a facility with staff professionally trained in treating sexual assault victims. Many communities offer sexual assault response teams that are called in to perform sexual assault medical and legal examinations. All personal belongings of the victim must be handled carefully. Rape examinations for most victims will be performed at no cost.
