A 67-year-old man recently lost his wife after being married for 42 years. He met his wife shortly after he returned from Vietnam as a combat soldier. He visits his physician and reports decreased appetite, moodiness, and extreme fatigue, even though he sleeps 10 to 12 hours a night. When giving his medical history, he admits that he drinks 4 or 5 beers and smokes marijuana almost every day.

The loss of his wife has caused the patient to slip into depression. In addition he is a veteran of the Vietnam War. Many survivors of the war suffer from post-traumatic stress disorder and anxiety.
The screening tools you can use are:
1. Geriatric depression scale - Short form: The patient answers yes or no to 15 questions. A score of 10 or greater indicates depression.
2. Short Michigan Alcoholism Screening test - Geriatric version: A questionnaire with 10 questions to which the patient has to reply yes or no.
The assessment data you need to collect as a nurse is we need to ascertain the real cause for the depression. Depression can be major or minor. The absence of neurotransmitters in the brain is the cause for the major type. In the minor type, the cause can be a loss or illness like cardiac disease, stroke and arthritis.
