The manager of the emergency department has created a team of personnel to reduce the number of accidents in the ED. Since, the patient has already received attention for the pain; the nurse need not attend to him urgently. Option B cannot be the right answer. The patient is a 30 year old with nausea and vomiting. Since the patient is now sleeping, the nurse need not attend to him urgently. Option C cannot be the right answer. Option D cannot be the right answer because the patient is a 28 year old with a wound in his arm and who is waiting for stitches. This patient would be the second option to the nurse. Hence, the correct option is A) a 60 year old patient waiting to go for an appendectomy.

Specific procedures which the team can implement to prevent medication errors are: 1. Check for medical bracelet 2. Check medication list or medication in luggage of patient 3. Obtain medical and family history of the patient 4. Specific charts for each person next to the patient and for the nursing staff A medical bracelet is sometimes given to patients who are mentally unstable. The medication list or medication gives us the details of the drugs already being used by the patient. The ED doctors can then prescribe accordingly. The medical and family history of a patient is very important. Since, the medical history tells of any allergies to specific drugs or foods, the doctor needs to know about the medical history and what procedures and tests have been done so far. The charts giving details of the patient should be very clear and the nurses should not confuse one patient with another if they are in the same room. Different colored charts can help in this case.
